Ice or heat is sometimes the million dollar question when a patient is faced with a new injury, re-aggravating an old injury or facing a chronic injury. The phase of the injury can determine when it is appropriate to apply ice or heat to the affected area, in addition to other factors (such as, where the injury occurred, underlying health conditions, etc.). As a Woodbridge Chiropractor, I often will see injuries from people in the workforce or athletic community. My experience has lead me to this conclusion. Not only is cryotherapy (or applying ice) great for acute injuries, but it is an excellent choice in the early weeks following surgery, or after re-aggravating a sub-acute injury that may suddenly swell up. Unfortunately, ice has gotten a bad rap due to some of the current research on muscle regeneration. Below is some valuable information to help you decide if you should use ice or heat.

The Importance of Ice in the Early Stages

Current research on using ice has determined that it can inhibit the production of certain processes that help regenerate muscle tissue and slow down the production of collagen. However, prolonged inflammation in an injured can cause the injury to stop healing and become a chronic problem. Therefore, we want to use ICE because it can help slow down the inflammatory process. Ice reduces inflammatory symptoms like pain and swelling through its effects on blood flow, nerve signal speed and tissue metabolism. Sometimes, the bleeding and inflammation process from an injury can cause additional damage to uninjured tissues near the primary injury site, causing “secondary injuries”. Icing an injury stops secondary injuries.

Now, you might be asking me, “will ice slow down my healing?” The answer according to the most recent literature is that icing will not cause any disruption to the healing process associated with injury management.

When is it Appropriate to Use Heat?

Both in my clinical experience and using the best available research, it is safe to say to begin using heat after four (4) days of ice therapy for mild injuries, seven (7) days for moderate injuries and ten (10) or more days after a severe injury or post surgical.

Applying local heat to an injured area after the swelling is stabilized has a number of health benefits. Local heat at about 68 degrees Fahrenheit helps to vasodilate blood vessels to encourage more blood to flow to the local area. Increasing blood flow to the area helps to bring nutrient and oxygen-rich blood to the injured area to promote healing. By increasing blood flow, we can encourage surrounding blood vessels and lymphatic channels to promote drainage of the injured area. And last – but certainly not least – increasing the temperature to the injured area can help boost the metabolism of damaged soft tissue, thus encouraging it to heal faster.

For Chronic Injuries such as arthritis, heat is a great and effective tool to use to soothe chronic aches in joints and muscles.

Class 4 laser therapy generates heat by causing a thermal effect in the affected area and increasing the metabolism of damaged tissue to help it heal faster. In my clinic, I often find that chronic pain responds well to a trial of care using Class 4 Laser by Summus Medical, and the heat the machine generates within the muscles and joints feels soothing and pleasant.

My rule of thumb for ice or heat therapy are as follows:

Ice: 10-15 minutes continuously every 2-4 hours

Heat: 15-20 minutes continuously every 4-6 hours

What are Good Sources of Ice or Heat?

From my clinical experiences and the results described by my patients, warm and moist heat always seems to work well. Products that seem to work well, have fair price point and last repetitive uses are the Magic Bag or any form of re-usable microwave heating pad. They help to draw ambient moisture and create a warm and moist heating environment for home use. As for ice, a re-usable gel pack is always a good option. They contour well to most body regions and can be used over and over again. A word of caution: never apply direct ice to any body region. Always place a thin face towel or a few sheets of paper towel to protect the skin from ice burns!

The first lens is manual therapy. A combination of manual therapy through chiropractic, acupuncture and rehabilitative exercise can greatly help to improve one’s musculoskeletal health. I have termed this: Healing from the outside in. But, another integral component in healing the body is healing from the inside out. Today’s topic of focus is bone health and ensuring our bone density remains strong. A strong skeleton will better support our body and yield better health overall.

Here’s Why You SHOULD Take D3 and K2

While most people are familiar with Vitamins A through E, Vitamin K isn’t spoken of much. Vitamin K is needed by the liver to create an enzyme that helps with blood clotting. Vitamin D3 works in so many incredible ways: it helps nerves transmit messages from the brain to the rest of the body, and to help the immune system fight off viruses and bacteria. Together, Vitamin D3 and K2 help to keep our bones strong and healthy.

Vitamin D3 and K2 work as co-factors: they help the body transport minerals through the body so the body properly deposits these minerals into our bones (where they belong!)

Vitamin D3 helps to make sure the intestinal tract properly absorbs calcium from our digestive system into our blood so it can make its way to our bones. However, too much calcium in the blood can get stuck, leading to heart attack and stroke. Here comes Vitamin K2’s role.

Vitamin K2 helps to activate a specific protein to prevent calcium from getting stuck in our blood vessels. Another important step in the role of K2 is to activate a protein called osteocalcin. This helps our body deposit calcium where it’s supposed to go: into our bones!

Often, wear-and-tear, trauma, sports injuries or carrying excessive weight causes the cartilage surfaces of our knee joint to become worn down. As this progresses, the joint becomes inflamed, leading to pain, swelling, stiffness, limping and impaired movement. Unfortunately, there is no cure for arthritis, but different treatment options are available to manage your symptoms, improve function and delay (or prevent) a joint replacement.

Often, a 3-4 week course of medical acupuncture, soft tissue therapy and chiropractic care will break the pain-inflammation-pain cycle and initiate a healing response in the knee. As patients begin to feel the effects of treatment, exercise is quickly added into the treatment plan. Exercise focuses on improving walking patterns, reducing limping, and strengthening the muscles supporting and surrounding the knee.

The long term benefits of any kind of treatment are locked in with a well-developed exercise program. By strengthening the muscles around the knee, you reduce unnecessary forces on the knee, resulting in less wear-and-tear.

In addition, turmeric, glucosamine and chondroitin are research-proven options to help preserve the remaining cartilage and keep inflammation levels low.

In my experience – and in keeping in line with the evidence by arthritis specialists – applying ice 6-8 times per day for 15 minutes for the first four days after an episode of pain can help reduce swelling and inflammation. Day 5 and onward, I recommend applying moist heat 6-8 times per day until symptoms are resolved. Moist heat can help to improve circulation and stimulate a healing response in the knee.

Do you miss your ergonomically correct office setup, supportive chair and dual monitors? Are you noticing that you are experiencing more neck pain, headaches, and tension in the upper shoulders?

If you are, there’s a chance that you’re developing the new Work From Home Postural Pain Syndrome. People with these symptoms usually experience pain in the neck, upper shoulders and even low back when sitting for long periods of time. Increased forward head posture, rounded shoulders and slouching in the low back contribute to these symptoms. The pain people often experience is from our postural muscles losing ability to hold poor posture for a sustained period, while joints are put in ranges they’re not used to. In some instances, muscle tension on the side of the neck can put pressure on the nerves traveling down the arm, resulting in numbness and tingling.

So, what causes Postural Pain Syndrome to occur you might ask? There are several reasons, but some of the most common ones we have identified are:

Poor biomechanics

Muscle deconditioning

Increased postural stress on your muscles

Ergonomics and Work set up

Stress and Anxiety

Reduced Physical Activity

Most times, the root cause of the problem isn’t us but the environment around us. As a Chiropractor, my goal is to help identify aggravating factors within your environment, provide ergonomic recommendations, and offer effective treatment strategies to help you feel better and stronger, faster! Oftentimes, patients respond well to a combination of Chiropractic Care, Deep Tissue Therapy, Ergonomic Modifications, and advice on Stretching and Postural Exercising. More shockingly, 9 out of 10 working Canadians can expect to experience low back pain during their lifetime. There is a significant burden on our health care system as a result of Canadians requiring medical attention for mechanical low back pain. When experiencing an episode of back pain, you might stop and ask yourself “why me?” or “why did this happen?” Most movement-related low back pain (or mechanical low back pain) is usually the result of a specific movement or movement pattern that has aggravated the low back, resulting in pain and dysfunction.

Besides receiving a clinical diagnosis, a key component to healing your back is to recognize what triggered your pain. By understanding what movement “threw your back out”, both you and your Chiropractor can take the necessary steps to identify the trigger, correct the movement pattern, and heal your back.

Exercises do not mean going to the gym for hours on end. These exercises are designed to be performed at home, requiring no equipment and no more than a 20 minute daily commitment. These are the “Big 3” back exercises that are essential for everyone. Whether you are suffering from sudden acute or chronic low back pain, these exercises are non-negotiable and must be performed daily!

You’re probably wondering why each set has fewer reps? By performing these specific exercises for low back pain in a descending pyramid, we reduce the chance of muscle cramps and injury, while still having an effective workout that is designed to target your pain. Over the course of a few weeks, you can begin to add more repetitions to these exercises as your back becomes stronger, feels less pain and you are able to move better. People who experience low back pain may benefit from a trial of chiropractic care before beginning rehabilitative exercises for their low back.

This week, the Ontario Government and the College of Chiropractors of Ontario has announced that chiropractors are allowed to resume regular chiropractic care. Routine maintenance chiropractic care, as well as continued care for acute injuries and emergency symptoms can now resume. I want to take this time to announce a few very important changes in the way the clinic will operate. Until the COVID–19 Pandemic is over, this will be our new normal.

As the clinic gradually re-opens, I ask my patients to follow the new mandatory clinic safety guidelines:

BOOKING YOUR APPOINTMENT

All appointments must be booked over the phone or through the Online Booking Website. Walk-ins will not be permitted at this time. Please be advised that I will be practicing on a limited daily schedule to ensure physical distancing and room hygiene is practiced. But, I will do my best to accommodate everyone’s appointment needs.

Intake Forms are to be completed online before your visit.

UPON YOUR ARRIVAL

Masks are mandatory and recommended. Masks do not need to be medical grade. Due to limited access to Personal Protective Equipment (PPE), masks will not be provided by us.

Arrive no more than 5 minutes prior to your appointment and please do not be late.

Please wash hands and/or sanitize prior to entering the clinic.

Practice physical distancing outside the clinic or waiting area until you are directed into a treatment room

PAYMENT AND INVOICING

Our credit and debit card terminals are equipped with tap-and-pay for tap-enabled credit cards and enabled smartphones

Treatment receipts will only be emailed. If you require a printed receipt, a monthly statement will be made available and prepared monthly.

CLINIC PROTOCOLS TO ENSURE YOUR SAFETY

Hand washing / sanitizing before and after every patient

Wearing PPE as necessary

Installing a sneeze-guard at the front desk

Disinfecting common areas often throughout the day

Spacing our patients so there are fewer patients in the clinic and allowing adequate time to clean

Having hand sanitizer available throughout the clinic

Screening all patients through the booking and arrival process

Reducing the number of “touch-points” you have in our clinic

If you begin to feel unwell leading up to your appointment, we ask that you contact us immediately to reschedule.

Anyone who has traveled outside of Ontario within 14 days, or who has been in contact with a suspected, probable or confirmed case of COVID-19 will be asked to follow mandatory 14-day self-isolation before booking an appointment.

If you would like to schedule your appointment online, click here. To schedule by phone: 905-856-4444

Certain dietary factors have been found to have immune-regulatory properties, such as Vitamin D and Omega-3 Fatty Acids. Often times, eating a well balanced diet (I prefer the Mediterranean Diet) still lacks some essential vitamins, mineral and essential components. Taking a nutritional supplement does not mean that you can eat a poorly balanced diet. Nutritional supplements are designed to “fill the gaps” in a well balanced diet to ensure that our body’s system is functioning at its best.

Omega-3 fatty acids are a part of the “Polyunsaturated Fatty Acid” family, with specific focus on ALA (found in nuts and seeds), EPA and DHA (the main components in fish oil). Byproducts of Omega-3 and Omega-6 fatty acids have important immune system functions, which fall under a specific category called “Pro-Resolving Mediators” (SPMs). These SPM’s are effective in reducing the inflammatory response caused by pathogens (including bacteria and viruses) and stress, and to help strengthen our immune system.

Omega-3 Fatty acids also help to strengthen our immune system by their ability to reduce inflammation and inflammation-related disorders. Probably one of the most renowned property of Omega-3 fatty acids is it’s ability to keep inflammation levels down in the body.

Vitamin D can help our body’s immune system recognize pathogens more effectively, thus producing a more accurate and heightened immune response when a pathogen is detected in the body. In addition, Vitamin D plays an important role in the uptake of calcium and bone health.

Regular physical activity has enormous health benefits, including: healthy weight management, improved sleep, less aches and pains, stronger hearts and less chance of chronic illness. Unfortunately, gyms and recreation centers are closed, resulting in people exercising at home or outdoors. Less traffic and warmer weather (eventually) on the way have resulted in many people people beginning to jog and run. As a great way to begin a journey to physical activity, there are a few things to keep in mind before you think about running the Boston Marathon. This article will help you understand a bit of running lingo, how to run more efficiently and enjoy pain-free running.

Common Running Terms

Learning to run means you have to learn a bit of running jargon:

Pace: How fast you’re running, usually expressed in kilometers per minute. Your pace will change every run depending if you are training for speed or distance, your effort, environmental conditions, and other factors. Don’t let pace be the defining factor in your run; as your fitness improves, you will have more consistent pace times.

Cadence: Also termed “Stride Rate”, cadence refers to how many steps you take in a minute of running. A higher stride rate (greater than 170) may reduce injuries and help you run faster.

Tempo Run: A sustained faster-than-usual run of shorter distance. These runs take more effort and more mental concentration, however they will improve your fitness without doing hard workouts.

Ten Percent Rule: A general rule of thumb for beginner runners. Don’t increase your mileage or intensity by greater than 10% each week. This is a good guideline to stay injury-free.

Warm-Up and Cooldown: A gradual way to run or walk at lesser intensity to prepare the body for your actual run workout. This will help to improve circulation, respiration and get blood to your muscles.

Common Running Injuries

It is common for a runner to experience injuries when they just begin running for fitness or in preparation for a marathon. Several injuries tend to impact the feet, ankles, knees and hips. Although these injuries may set you back a bit in your running progress, it is crucial to treat these injuries before they become a chronic condition:

Runners Knee: A common condition that is defined by generalized pain around the front of the knee joint. Usually defined as a dull ache around the front and behind the knee cap, runners knee is a common injury that can result from muscular imbalances in the thigh, inefficient running patterns or wear-and-tear behind the knee cap.

Iliotibial Band Syndrome: Also known as “IT Band Syndrome”, this thick tissue along the outside of the leg can become inflamed and painful resulting in pain along the outside of the hip, thigh and knee.

Achilles Tendinitis: Repetitive overuse of the Achilles Tendon (the thick rope-like tendon behind the ankle) can result in inflammation and pain at the back of the leg and calf. Most runners may begin to experience this if they suddenly increase their distance or intensity in their runs.

Shin Splints: One of the most common running injuries, shin splints feel like razors along the front of the tibia (shin). Shin splints can result from lack of stretching, poor footwear, flat feet or rigid arches.

What Can You Do to Prevent Running Injuries?

To determine if running is the right form of exercise for you, it is very beneficial to consult with your healthcare professional to have a baseline of your physical fitness. Baseline measures can include heart rate, blood pressure, body composition and health factors that may pose a risk for you as an aspiring runner.

A chiropractor can help you with running by using your baseline health measures, health history and personal / lifestyle factors to help design a running program. Further, a chiropractor can help you with stretching, foam rolling, nutritional advice and determining if orthotics are right for your running shoes.

If you are an athlete, then you are familiar with the physical demands of participating in sports, such as running, skating, and jumping. When you put your body through these activities, it can result in painful tears and strains to your muscles. Regularly visiting a chiropractor can not only help you recover from sports related injuries, but will also help prevent them.
Chiropractors are able to treat and prevent injuries in your back, neck, shoulders and knees. They are knowledgeable in the manual adjustments of the spine, which ensures that your bones are properly aligned and help relieve tension in your body. Visiting a chiropractor has multiple long term benefits, such as allowing your body to function better and become less susceptible to sports injuries.
The main causes of sports related injuries include forceful impact, repetitive training, and training without a proper warm-up routine. Chiropractors help athletes relieve any pain they may be experiencing and help them recover faster after an injury.

What To Expect At Your Appointment

When you go in for your appointment with your chiropractor, they will begin by asking you a few questions about your overall health and perform a structural examination. During the structural examination, they will examine your back, ligaments, and joints. With this information, they will be able to come up with a treatment plan that will address your areas of concern.
The two most commonly used techniques are spinal manipulation and spinal mobilization. With spinal manipulation, your chiropractor will put you in a different position and do a manual impulse to the affected joint. This will help to release any pent up pressure in the joint and encourage your muscles to realign so that they are able to operate optimally.
Spinal mobilization is often used for patients that need a more gentle approach to restore their joint functionality. With spinal mobilization, chiropractors use tools and stretching to help release the pressure in the joints.
Many times, chiropractors will add other treatment methods, such as ice, heat, acupuncture, laser therapy and electric stimulation to help relieve their clients’ pain, depending on the nature of their injury. It is very important that you openly discuss your symptoms and overall health in detail with your chiropractor so that they can determine the best treatment options for you.

How Do You Prevent Sports Injuries?

When it comes to sports injuries, it is important to remember that injury prevention is key.
Living with physical or mental chronic pain can be taxing. With a myriad of different invasive, drug-based pain treatments available, acupuncture is a viable alternative for individuals that desire a more natural approach. Acupuncture therapy is conducted by a licensed acupuncture provider who will apply needles to various parts of the body, especially areas causing pain, which releases endorphins. Endorphins are our body’s natural pain-killer, which work to promote circulation to the injured area and promote stimulation within the immune system.
Read on to learn more about how acupuncture works and the specific health conditions and chronic pain conditions it can treat.

Most acupuncture patients access treatment for chronic lower back pain, migraines, tension headaches, and knee pain. These common health conditions can make everyday tasks much more difficult. Acupuncture can alleviate some of the pain and promote a sense of normal functionality. Here’s how acupuncture benefits these health conditions:

Lower Back Pain: Acupuncture has been associated with decreasing chronic pain or the intensity of the pain in the lower back area to promote better functionality in a short period of time. Acupuncture stimulates acupoints for the lower back pain to relieve chronic pain. Altering brain chemistry by prompting the release of neurotransmitters can stimulate nerve impulses to reduce the sensitivity for pain.

Migraines: Acupuncture is often used to treat migraine symptoms and prevent migraines from returning. The treatment can stimulate nerves in the body to release endorphins, which promote a flow of good energy. A healthy immune system can relieve symptoms of migraines. Alternative, natural pain treatments, and therapies can reduce the frequency of returning migraines. Increasing blood flow and circulation by applying needles to the head area can prevent the long-term effects of migraines.

Knee Pain: Acupuncture can improve knee function in the short-term or long-term. Chronic knee pain, often caused by osteoarthritis, can severely impact the day to day lives of individuals, especially seniors and women. Acupuncture can mitigate pain and manage it by stimulating acupoints around the knees and promoting the “qi” energy throughout the body. The “qi” energy is a vital energy source that promotes good health and vitality.

What To Expect During Acupuncture Treatments

During acupuncture treatments, the acupuncturist will examine you to understand your health condition. You will be asked to lay down either on your side, front or back and sterile, thin needles are applied to specific acupuncture points based on your areas of pain. Sometimes, gentle stimulation is applied to the needles to enhance the effects of the acupuncture session. Treatment duration can last anywhere from five to 30 minutes. Dr. Mathew Saturnino is a Vaughan wellness clinic that specializes in innovative, natural treatments, assessments, and diagnostics.
